Texas Health Resources serves patients in North Texas. If you did not receive your COVID-19 vaccine, or COVID-19 test at a Texas Health location, we will not have your results or vaccine record. Contact the organization where you received your vaccine or test for help.
To create your MyChart account, please fill out the form below.
Please note: To create an account, the information you provide must match the information in your Texas Health medical record. If the information does not match, a MyChart representative will contact you within two business days.
